CCD C-CDA |
Continuity of Care Document. Also referred to as a ‘Clinical Summary of Care Record.’ The CCD is a summary of the pertinent clinical, demographic and administrative data and provides a ‘snapshot in time’ for a specific patient. |

Unsolicited Observation Results. ORUs are often used to convey clinical reports such as lab results, radiology reports, EKGs, vital signs. |

Rad |
Radiology Reports. Radiology information is typically a transcribed report resulting from the Radiologist’s dictation of their interpretation of an imaging study (chest x-ray, CT scan, MRI, etc.). |
